All the mandatory ticket fields you have set up under Admin > Ticket fields will be displayed under Feedback widget. So navigate to Admin > Ticket fields and change the name of the 'Requester' field to 'Your name' and it will reflect on your widget as well.
In the feedback widget, I want to change the help text of email from requester to your email
