If you signed up before Aug 9, 2021, please click Previous plans to view your applicable plans.
This may be because the field isn't set to be displayed to the customer.
To fix this, please navigate to Admin > Workflows > Ticket Fields and click on the field that is not visible on the form. On the dialogue box that opens up, select the option 'Display to Customer' and also 'Customer can edit'.
This will ensure that the fields are visible on the ticket form from the customer’s side as well.
